APPOINTMENTS

We work by appointment only. Please call to schedule appointments.

Should you have more than one child needing a medical evaluation, an appointment is needed for each child.

Please arrive on time for your appointment and be prepared to verify your current address and responsible party. Be prepared to show your photo ID.

You will be required to pay for all services provided, and any outstanding balance, at the time of your visit.

INSURANCE

We do not accept insurance.
We provide a Superbill so you can file with your insurance provider. We also provide a limited amount of free consultation on filing your insurance claims.
If your insurance carrier requires additional or unusual paperwork, there will be a fee charged for this service. This would include letters of medical necessity and/or appeals.

Script Refills

Coming to our office to get a controlled substance prescription often means taking a considerable amount of time off of work or school. We have completely eliminated this problem by expanding our mail delivery.

All refills must be requested at www.georgetownkids.com. The scripts tab is at the top of the website. While making your script request, you will be able to ask for a telephone consult with Dr. Hossalla.

Medication management/controlled prescription refills require chart review, patient evaluation, possible phone calls to the parent/school, and a review of the treatment plan for every refill. We request five business days to complete these steps.

We will normally deliver scripts by USPS regular 1st class mail. If you do not wish to wait for five days processing, we have expedited service available.

We hope you find online script ordering and home/office delivery convenient and worthwhile.

When sending controlled substance prescriptions directly to pharmacies is readily available in Texas, we will make the switch at no additional cost to you.

OFFICE ETIQUETTE

Turn off all cell phones and other media while in the office and during the visit with Dr. Hossalla. Dr. Hossalla does not use her time to conduct personal business, but is focused on your child.

Bringing in food, drinks, and snacks is prohibited. We make it a priority to keep the office clean and free of bugs, etc. We need your help to keep it that way.

Please keep children seated if possible for their own safety. Unruly behavior increases the chance of accidents and injuries and is discourteous to others. We would rather they have their accidents elsewhere.

COPIES OF MEDICAL RECORDS

We charge $25 to retrieve and copy medical records. If you are moving or need a copy of your child’s medical records for other purposes, please make your request at least two weeks in advance.
A release form is available online.

BILLING STATEMENTS

Statements from this office are due upon receipt.
