Although we do not file your insurance for you, we do provide you with a Superbill so you can file with your insurance company. Most insurance filing forms are on our website.

$300 initial comprehensive fee, includes:

• Initial visit
• Medical exam
• evaluation
• All communications
• Prescribe medicine (if necessary)
• Monitor and adjust medicine
• Pre-Authorization for medicine
  (not guaranteed as insurance policies differ greatly)
• 504 correspondence to schools
  

$150 Follow-up Visits (usually 2 visits per year), includes:

• Health Check-up
• Adjust medication
  

$39.85 Normal prescriptions, (up to 5 businesss days)
56.17 Expedited presciption (1 business day)

After your medicine has stabilized, you will order your scripts on our website. We consider medicine stabilized when we have established an effective medicine and dosage.
