Do I always need to call in advance for an appointment?   Yes

  
   Please, when you call for your appointment:

• Are you are calling for an initial or follow-up visit?
• What is the specific reason for the visit?
• Any and all children you expect to be seen.
• Do you feel you need a same-day appointment?
• Make your 2,4,6,9,12,15,and 18 month appointments at the time of other visits if possible.
• Schedule yearly checkups and pre-participation
  sports physicals one month in advance.

• Explain to the receptionist that you wish to make a first-time visit.
• Please arrive 30 minutes early to complete new patient paperwork.
• Bring identification and current copy of your medical insurance card.
• Bring a list currently prescribed medications, with their strength and dosage.
• Bring your child's current record of immunizations

• Please arrive on time for your appointment.
• When you arrive, inform us of any insurance, address, telephone, or responsible party changes.
• We will ask to see your insurance card.  Have this available and handy.

• Write down any questions or concerns that you would like to discuss with your physician.
• When visiting with the physician, tell the physician when your symptoms first started,
  and what you have done to take care of the problem.
• Always ask your doctor or nurse to explain anything you don’t understand.
• Take notes. Writing down instructions will help you remember them after you leave.
